Responding to the Challenges of Patient Care Integration, Adherence and Follow-up among the Urban Poor

Obejctives 1) Present the case studies of the LA Christian Health Center Outreach and/or clinic operations and that of Oscar Romero Clinic. 2) Identify the key clinical issues of Patient Care Integration, Adherence and Follow-up among the dynamic urban poor population. 3) Identify evidence-based models of response (i.e. Kaiser, etc.) for clinicians (MDs, RNs) 4) Provide clear next steps and resources to equip the attendees.
