3. EXHIBITOR REGISTRATION Please see the EXHIBITOR PAGE. Exhibitors please review the Exhibitor Guide and complete the Exhibitor Registration form before proceeding with payment. (You may use the e-shop link in paragraph 4 of the Exhibitor Registration form for payment)
     Includes a table for Friday and Saturday and registration for one Exhibitor for Thursday, Friday and Saturday Conference. Also includes CME/CEU/CE credits, Thursday night dinner, Friday and Saturday lunches and snacks. EXHIBITOR TABLES will be available for FRIDAY and SATURDAY ONLY.

PayPal Although PayPal is processing all conference credit card transactions, you do not need to sign up for PayPal. PayPal will bill you $1.00 for processing. There are no other shipping and handling charges.

In the event that you cancel your registration on or before September 24, 2012, a refund minus a $15 cancellation fee will be returned. No refunds will be credited for cancellations after September 24, 2012. Please allow 30 days for processing.

It is possible to change your registration category at the conference without any charge.
